(76) Multiple Giant Nabothian Cysts in a 14-year-old girl: A Case Report

Background: Nabothian cysts are benign cysts of the cervix that develop due to blockage of mucus-secreting glands in the endocervix. While these cysts are commonly seen in adult females, the literature lacks reports of giant Nabothian cysts in adolescent girls. This case is significant as it highlights the occurrence of giant Nabothian cysts in a pubertal female, emphasizing the need for awareness and evaluation in younger populations.

Case: A14-year-old gravida 0 female presented for management of large Nabothian cysts. She had a 6-month history of severe epigastric and pelvic pain, constipation, pain with defecation, nausea and anorexia leading to numerous ED visits. She underwent GI evaluation, and was initially diagnosed with fatty liver, gastritis, and ovarian cysts. However, a repeat ultrasound at a subsequent admission for lower pelvic and back pain revealed multiple, large cervical cysts. MRI confirmed multiple giant Nabothian cysts with the largest measuring 4.5 x 4.7 x 3.0 cm and the aggregate size measuring 4.5 x 4.7 x 4.0 cm.

Comments: This case represents the first report of rare, giant Nabothian cysts in a pediatric patient and underscores the importance of using MRI and gynecological surveillance when assessing pelvic pain in pubescent females.
